E.2 · Documentation and Reporting

Seeking clinical direction

Follow examples of asking a precise question when instructions are unclear or conflict, then confirming and documenting the answer. Review how urgency determines the contact route and how implementation stays within the current plan and demonstrated competence.

Task E.2: Seek and prioritize clinical direction from a supervisor promptly

Useful foundations: F.2: Demonstrated competence

The rule

When instructions conflict or leave a decision unclear, ask about that exact step, use the appropriate urgency and confirm the direction received.

State the exact decision that needs clarification. Use the designated coverage route when the primary supervisor is unavailable, and document the direction received.

Key terms
Coverage
The designated alternative clinician or contact route when the usual supervisor is unavailable.

Worked examples

Conflicting instructions

Identify the disagreement

The current printed plan says to wait three seconds before a gesture prompt. A new message says “use a five-second delay,” but does not identify which program it applies to. The RBT is about to run that skill.

Relevant facts

  • Two different delays appear in the materials.
  • The new message’s program is unclear.

Ask which instruction applies and follow the organization’s process while awaiting clarification.

“Does the five-second delay replace the three-second delay for this program?”

Ask which program the five-second direction applies to. While awaiting clarification, continue activities that can safely follow clear current instructions.

Act on guidance

Close the communication loop

The supervisor confirms that the five-second delay applies to the named program from today and sends the updated plan. The RBT has already been trained in its prompting procedure.

Relevant facts

  • The program and effective date are explicit.
  • The updated procedure is within trained competence.

Confirm understanding, use the updated plan and record the change through the required process.

Clarification → confirmed current plan → consistent implementation

After obtaining clarification, update the active materials through the approved process so staff use the same current instructions.

What changes the answer?

Keep the supervisor’s routine inbox unanswered; change the situation.

  • The RBT cannot yet obtain a routine reply.

A nonurgent instruction is ambiguous.

Use the designated clinical coverage process.

Obtain the missing instruction through the designated supervisory route.

A safety emergency is occurring.

Activate the emergency protocol and urgent contact route.

Use the emergency route promptly to obtain immediate protection.

Use the urgency of the situation to select the appropriate route for help.

Remember

  • Name the exact step and the instruction that needs clarification.
  • Confirm and document the answer so the whole team uses the same instruction.
